Activities & session tools
Small steps you can work through together.
Use these interactive sheets to structure a conversation, plan an action and reflect on what you have tried. You can print a blank or completed sheet, or download your notes as a text file.
Before you start
These are optional working sheets, not an assessment. Answers stay in this page and are not sent to Aspect or stored by the website. Use example answers when reviewing the design. Avoid names, case notes or sensitive information on a shared device, and clear your answers when finished.
A downloaded or printed copy is yours to manage. These tools do not save into a staff case-management system or establish that a support session took place.

Planning an onward referral

Describe the outcome you want, without including sensitive case details.
Use the partner directory to find the official service information.
Eligibility, current availability, access needs and the right referral route.
Agree what would be shared, who would receive it and how to use the approved route.
For example: support worker to check the current referral guidance.
Agree how to check whether a response has arrived.
Resident-facing aids, not support records
These tools are for residents to explore ideas and plan everyday actions. Do not use them to save or store evidence of support sessions. Staff evidence stays in the approved case files and Caseload. Printed or downloaded worksheets are personal working copies, not approved case records.
